Providing exceptional, comprehensive care to maintain independence for our area’s seniors is the united goal for PACE Southeast Michigan. PACE stands for Program of All-Inclusive Care for the Elderly and serves individuals who are age 55 or older, certified by their state to need nursing home care, able to live safely in the community at the time of enrollment and live in a PACE service area. While all PACE participants must be certified to need nursing home care to enroll in PACE, only about 7 percent of PACE participants nationally reside in a nursing home. If a PACE participant needs nursing home care, the PACE program pays for it and continues to coordinate the participant’s care. Founded in 1994, PACE Southeast Michigan, a non-profit organization jointly owned by Henry Ford Health System and the Presbyterian Villages of Michigan, serves seniors at six locations — Dearborn, Eastpointe, Pontiac, Southfield, Sterling Heights, and the Thome Rivertown Neighborhood in Detroit. The program brings together all of the care needed including medical, social, behavioral, nutritional, pharmaceutical, rehabilitative and transportation services in a truly integrated model where the interdisciplinary team builds a strong relationship with each participant. The support and services we provide allow older adults to remain independent in their own homes for as long as possible. Our interdisciplinary team proactively communicates and coordinates holistic care to prevent avoidable emergency visits, hospitalizations and nursing home placements, thus providing higher quality care. As a PACE Southeast Michigan participant, all health care services are provided and arranged by your personal health care team. PACE participants may be fully liable for the costs of medical services from an out-of-network provider or without prior authorization with the exception of emergency services.
