The right match can make a real difference! 💙

In this video, Jamsel Reyes, a member of BRIDGES’ Community Habilitation team, sits down with Anthony to hear about his experience with the program and how individualized support has helped him build confidence, independence, and meaningful connections in his community. 💫

Before receiving Community Habilitation services, Anthony had limited opportunities for individualized support focused on building independence and becoming more active in his community. Finding the right fit was an important part of his journey. Jamsel took the time to get to know Anthony—his personality, interests, strengths, and goals—helping build the trust and positive relationship that made their work together possible. 🤝

Together, Anthony and Jamsel have worked on socialization, communication, community participation, daily living skills, and greater independence, while creating opportunities around the activities and interests that are meaningful to Anthony. 👥

At BRIDGES, Community Habilitation is about more than providing a service. It’s about seeing each person as an individual and helping them build confidence, pursue their goals, and have more opportunities to succeed in their community. 💪

Interested in making a difference in someone’s life? We’re looking for Community Habilitation Specialists! To learn more, email our program coordinator Benjamin Hay at [email protected] today. 📲

The Hudson Valley E-MDT serves Dutchess, Orange, Rockland, Sullivan, and Ulster Counties, working together to help protect older adults from abuse, neglect, financial exploitation, scams, and other situations requiring coordinated support.

If you are a participating professional or agency and would like to learn more about making a referral, contact Raymond Webb, Hudson Valley E-MDT Coordinator, at 845-624-1366 Ext. 200 or email [email protected]. 📲
