The weather turns and plenty of people feel it before they can say what it is. Slower mornings, an appetite that shifts, sleep that moves an hour earlier or later without being asked.

When someone comes in at this time of year, three things go on the page first: what sleep has actually been doing, what has changed in the last month (food, travel, work, light), and what energy does between three and six in the afternoon.

None of it is a diagnosis. It’s the start of a conversation, and a much better one than “I’m just tired”.

Weeks, months, or a year?

When someone tells me they’re not sleeping, that’s one of the first things I ask, because the answer changes almost everything that comes after it.

A rough fortnight after a hard month is usually a normal response to a hard month. It tends to settle on its own.

Months is a different thing. By then it has stopped being a response and started being a routine.

And a year is a pattern. Patterns have reasons, and they’re usually worth looking for.

What I see most is people waiting far longer than they needed to, because it never quite felt bad enough to bother anyone about. There’s no threshold you have to cross first. Your GP is a good place to start, and naturopathy can sit alongside that care.

Asleep by ten, wide awake at three, and it only started in the last year or two.

It’s one of the patterns I hear most from women in their forties, often before anything else seems to have changed. As hormones begin to shift, sleep can get lighter in the second half of the night. Nothing dramatic wakes you. You’re just easier to wake, and it’s harder to drift back.

A few things tend to sit alongside it: feeling warmer at night, a mind that switches on the moment you’re awake, and a glass of w**e that never used to matter now showing up at 3am.

It’s common. That doesn’t make it something you just have to live with. Your GP is a good first stop, and naturopathy can sit alongside that care. In a consult we look at the whole night, not just the wake: evenings, cycle, stress, and what you’ve already tried.

Most people who come to me about sleep don’t only have a sleep problem.

Sleep sits in the middle of everything else. It’s often one of the first things to go when hormones start shifting in the forties, and often one of the last to come back. It moves with stress, in both directions. And it tends to run both ways with digestion: an unsettled gut can wake you, and a rough night can make the next day’s digestion harder work.

So sleep is often the thing people finally book about. It’s rarely the only thing we end up talking about, and that’s not a detour. It’s usually where the useful part of the hour is.

It’s a good door in.

Passionflower (Passiflora incarnata) is a climbing vine from the Americas, and one of the more specific herbs in the Western dispensary.

Specific, because traditionally it isn’t reached for for tiredness, or for sleep in general. It’s used for one particular problem: a mind that keeps circling. The person who is exhausted, lying down, and still running the same three thoughts.

That distinction is most of the skill. Three people can say “I can’t sleep” and need entirely different things. One can’t switch off, one wakes at three, one is never tired at the right hour. Same complaint, different herb.

Which is why “what should I take for sleep?” almost always gets another question first.

This one comes up more than the 3am wake, and people are always slightly embarrassed by it.

Nine o’clock on the couch, gone. Asleep mid-sentence. Then you get up, brush your teeth, walk past three lights, check one thing on your phone, get into bed — and you’re wide awake, it’s eleven, and now you’re annoyed as well as tired.

The usual explanation is that you “missed your window”, which isn’t quite it. Your evening takes its cues from movement and light far more than from what you intended. Standing up and walking through a lit house reads, fairly enough, as a decision to keep going.

Which is why this pattern tends to be one of the more workable ones. It’s rarely about the sleep itself. It’s about the fifteen minutes either side of it, and that’s a much easier conversation than “I can’t sleep”.

Tired most of the time, and the blood test came back fine?

With sleep and energy there are three results I always want to see: iron, thyroid and B12. Not the word next to them. The actual numbers.

For iron that means the stores as well as the haemoglobin, since ferritin is what shows how full the tank is. For thyroid, a TSH on its own is one part of the picture, so it helps to know what else was measured and what wasn’t. And B12 is simply worth seeing written down.

Each of the three can turn up as tiredness, which is why I’d rather look at them side by side than one at a time.

None of this replaces your GP. Results are a conversation with them too, and if anything needs a doctor’s eyes, I’ll say so plainly.

Bring the printouts. Old ones too. Two and five years ago tell me more than one.

You got to sleep fine. Four or five hours later you're completely awake, and the hour is oddly consistent, the same one, most nights.

That's rarely a sleep problem arriving late in the night. More often it's the day before turning up at the one hour it can finally be heard. Blood sugar, stress hormones and body temperature all move through that same window, and what happened between three in the afternoon and ten at night shapes how that window goes.

So the first questions aren't about sleep at all. What did you eat after six? What was the last hour of the evening actually like? And how long has three o'clock been your hour? Six weeks and six years are two different conversations.

None of this is a character flaw. It's information, and it's usually the most useful thing anyone brings into a first consult.

You came in about your gut. So why am I asking about your sleep?

It's the question people find oddest in a first consult, and it's usually the one that changes the answer.

The plain version: the gut and the nervous system share a line, and neither one waits politely for the other to finish. What I hear most weeks is some version of "fine at breakfast, wrecked by three — and I haven't slept properly since June." That's not two problems. It's one system, described twice.

Which is why a food list is rarely the first thing I reach for. Cut enough out and you end up with four foods and the same symptoms, which is a worse place to be than where you started.

I'd rather start with four questions, none of them about food. They're the ones that tend to reorganise a picture that's felt random for months.

If your gut and your sleep both went off around the same time, that isn't a coincidence — and it's worth an hour.
