Where to start?
This year’s theme is about making space for people to be heard. And sometimes, that starts with the one conversation you’ve been meaning to have, but haven’t quite got around to. A few simple ways to make sure it happens:

→ Ask the second question.
Don’t stop at “How are you?” If something you’ve noticed makes you wonder, ask about it gently.

→ Follow up.
If someone opens up, make a note to check in again. Put it in your calendar if you need to—a small reminder can help a meaningful conversation continue.

→ Be prepared.
Save useful support numbers in your phone now, so they’re there if you or someone you know needs them later.

If you’re looking for personalised guidance, a Telehealth consult with our experienced clinician can help. .health medical professionals are attentive, caring and compassionate.

When someone opens up, the instinct is to fix it. That’s rarely what they need in the moment, and reaching for a solution often ends the conversation before it has properly started. R U OK’s guidance is to listen without judgement, take what's said seriously, and resist the urge to solve. Sitting with what they’ve said, and asking what’s been going on, will do more than any answer you could offer. Nobody expects you to be a clinician, and you don’t need to be. Noticing, asking, listening properly and staying in touch is the whole of it. The message a week later usually matters more than anything either of you said on the day.

Sleep, movement and connection are not a substitute for professional support, and nobody should be told to walk it off. But the evidence behind them is solid. The Black Dog Institute puts the reduction in risk of major depression associated with regular exercise at 26%, and identifies consistent sleep timing as one of the more reliable things a person can control. They’re also the first things to slip when someone is struggling, which is exactly when they’re hardest to keep up. Doing the thing with someone tends to help more than suggesting it. Going for the walk with them. Picking up their shopping. Sending the message that doesn't ask for a reply.

Low mood is only one presentation and often not the most visible one. Beyond Blue lists increased irritability, withdrawal from activities and people, changes in sleep or appetite, avoidance, and difficulty making decisions among the common signs. Drinking more and working too much are common too, and men especially tend to notice anger, irritability or something physical long before they’d call it sadness. It matters for who gets noticed. Australian men see a health professional for their mental health at roughly half the rate of women, and some of that gap is recognition rather than need2. If someone has changed, and stayed changed, it’s worth asking about. However that change happens to look.

Most people answer the first question with some version of “fine”, because that’s what the question invites. The second one is where the conversation actually starts. Asking again, with something specific attached, is how you show the question was real. You’ve noticed they’ve been quiet lately. You noticed they skipped the thing they never skip. Saying what you noticed gives them something real to answer, and makes it clear you’d rather hear the truth than the polite version.

World Mental Health Day falls on Saturday 10 October, and October is Mental Health Month across most of Australia.

This year’s global theme is Lived Experiences Heard: Real Voices, Real Change. It centres the voices of people with lived experience of mental health challenges, and the importance of making space for those voices to be heard. Sometimes what helps happens in conversation with someone who isn’t a clinician.

Our October newsletter covers what that looks like in practice. Why the second question matters more than the first. Why struggling often shows up as irritability or going quiet rather than sadness. What to say when someone opens up, and what a Mental Health Treatment Plan from your GP actually gets you.

If there's a conversation you’ve been meaning to have with someone, this is as good a week as any.
