Three out of four diagnoses come from the patient's story, not the test.

I recently explained this to a room of first graders. A doctor is a body detective. Ask, look for clues, then help. They understood it instantly.

As an internal medicine physician in Bethesda, I build longer visits for exactly this reason. The answer is usually in what you tell me.

We had this conversation live last Friday with Dr. Kelly Donahue, PhD, a clinical health psychologist. These are the moments that stayed with me.

Weight gain in midlife is not simply about willpower. It is a window of about 4 years when the rate of fat gain doubles and lean muscle declines. This is not ordinary aging. It has a start and a finish. Stress changes appetite, impulse control and decision-making, so the same eating and exercise can stop working. And a standard physical rarely checks the labs that help explain it: fasting insulin, a full thyroid panel and vitamin D.

The full recording and the Weight Gain Lab Checklist are available for everyone who registered. If you weren’t there, get the full recording in the first comment.

Today, I spoke to the women of the Greater Bethesda Chamber about running your health with the same rigor you run your business.

I closed with four questions. Not about my practice. About yours, wherever you get care.

Most of these never come up in a fifteen-minute annual visit. That is a reason to ask, not to skip them.

Save this for your next appointment. If you're in the Bethesda area and want a doctor who asks them for you, you can schedule a discovery call. Link in bio.

Before I opened my practice, I spent eight years as a hospitalist at Sibley, including as medical director of the orthopedic floor.

A hip fracture is rarely just a broken bone. It starts a cascade: blood clots, weeks of immobility, confusion, depression. About 1 in 5 people die within a year, and 40% can't walk independently a year later.

Most of those fractures were years in the making. That's why I check bone density earlier when a woman has risk factors, not at 65 by default.

Birth control in your 40s looks different than it did at 20, and most Bethesda primary care visits never revisit it. "Am I too old for birth control?" I hear this often.

According to the CDC's 2024 contraception guidelines, age alone doesn't rule out any method. What matters is your health history: blood pressure, migraines, smoking, clot risk.

Dr. Anne Burke of Johns Hopkins covered this at the Perimenopause Symposium. Some methods do double duty, easing heavy bleeding and perimenopausal symptoms.

1 in 2 women over 50 will break a bone from osteoporosis, and in my direct primary care practice in Bethesda, MD, preventing that starts years earlier.

As a hospitalist, I saw where the gap in women's preventive care leads: a fall, a broken hip, an infection that turned into sepsis, often years after no one screened for the risk.

Two guideline backed steps help close that gap: bone density screening at 65, or sooner if you're at risk, and treatment for recurring infections after menopause.

Today is World Osteoporosis Day. If you've never had a bone density scan, ask why.

𝟳𝟭% 𝗼𝗳 𝘄𝗼𝗺𝗲𝗻 𝗶𝗻 𝗽𝗲𝗿𝗶𝗺𝗲𝗻𝗼𝗽𝗮𝘂𝘀𝗲 report joint or muscle pain. Most are told it's just getting older.

Today is 𝗪𝗼𝗿𝗹𝗱 𝗠𝗲𝗻𝗼𝗽𝗮𝘂𝘀𝗲 𝗗𝗮𝘆. The International Menopause Society chose "Chronic Pain at Midlife" as this year's theme, and its new White Paper says this pain is under-recognized, underdiagnosed, and undertreated.

Before I opened my practice, I was Medical Director of an orthopedic surgery floor. I saw how often midlife pain was treated one joint at a time, without anyone asking what else had changed that year.

Joint pain, migraine, pelvic pain, and poor sleep often show up together in midlife. They deserve to be looked at together, with time, a full history, and an honest plan.

Comment 𝗦𝗬𝗠𝗣𝗧𝗢𝗠𝗦 and I'll send you my free Perimenopause & Menopause Symptom Breakdown.

Menopause care shouldn't be a niche, and in my direct primary care practice in Bethesda, MD, it never is.

Women get steady care through the childbearing years, then the gap often fills with supplements and claims that have no evidence behind them.

In my practice, menopause care is primary care: symptoms taken seriously, hormone therapy options discussed, and heart and bone health tracked in one place.

I talked about why I built my practice around this gap on On Her Way with Gitika Kaul; the episode is live now.

Eating the same. Exercising the same. Still gaining weight?

I hear this from my patients in their 40s and 50s all the time. There is a specific window in midlife when body composition shifts, even while the scale barely moves, and most routine labs don't catch it.

On Friday, October 23 at 12:00 PM ET, I'm talking about it live with Kelly Forys Donahue, PhD, a clinical health psychologist. It's free, and everyone who registers gets our Weight Gain Lab Checklist, even if you can't attend live. Registrants also receive the recording.
