One Medication List. Fewer Surprises.

Prescriptions change. Specialists add new medications. Hospital stays can bring new instructions. Over time, a medication list can become confusing - especially for older adults managing several health conditions.

National Check Your Meds Day is observed in October and is a reminder to review everything you take, including prescriptions, over-the-counter medications, vitamins, and supplements.

Mobile Medical providers can review medications where eligible patients live, reconcile medication lists, look for possible duplication or side effects, clarify recent changes, and communicate with the patient's broader care team when questions arise.

A medication review is not about stopping medicines on your own. It is about making sure each item still has a clear purpose and that the full care team is working from the same list.

Ask Mobile Medical about an in-home medication review.

Strong Bones Support Independent Living

Osteoporosis can develop quietly for years, often without obvious symptoms until a fracture occurs. For older adults, a fracture can affect mobility, confidence, independence, and the ability to remain safely at home.

World Osteoporosis Day is a good reminder to talk with a healthcare provider about bone health. A primary care visit can help review risk factors, medications, fall history, nutrition, mobility, and whether bone-density testing or additional evaluation may be appropriate.

Mobile Medical can help eligible patients address these concerns where they live, coordinate appropriate testing and follow-up, and connect patients with additional services such as home health or therapy when clinically indicated.

Protecting bone health is part of protecting independence.

Better Transitions Start With Great Case Managers

A smooth discharge takes more than a set of instructions. Hospital and SNF case managers help patients and families understand what comes next, coordinate services, identify barriers, and connect people with the right resources across multiple care settings.

Mobile Medical is proud to work alongside case managers as a referral partner for eligible patients returning home. Our in-home primary and specialty care services can help support follow-up, medication review, acute and chronic concerns, and communication with families, home health, specialists, and established providers.

Case managers carry an enormous amount of knowledge and responsibility - and their ability to connect patients with the right resources can make the transition home safer, clearer, and more connected.

During National Case Management Week, we thank the professionals who help patients navigate every next step.

💐 perfect way to brighten the day at Brookdale Mandarin Central!

💐 perfect way to brighten the day at Brookdale Mandarin Central!

💐 Thank you to Marcie Metcalf and Brookdale Mandarin Central for hosting such a wonderful Build Your Own Bouquet event!

There’s something special about flowers. Their colors catch our eyes, their fragrances can bring back memories, and the simple act of arranging them gives us a chance to slow down and enjoy the moment. Engaging our senses can spark positive emotions and create meaningful connections.

That makes events like this especially meaningful in an early-stage memory care community. Familiar sights, scents, and hands-on activities can help residents connect with memories, express themselves, and enjoy time together.

This one hit a little closer to home for me, too. 💐 My daughter was speechless when I brought her home a bouquet. I showed her how to trim the stems, fill a vase with water, and find the perfect spot for her flowers right on her bedroom dresser. Watching the joy something so simple brought her was incredible.

It was a good reminder that sometimes the simplest things bring the most joy. Whether its a toddler proudly displaying flowers in her room or a resident enjoying a familiar scent, these little moments can mean so much.

Thank you again, Marcie and Brookdale Mandarin Central, for creating opportunities like this for residents, families, and professionals to connect! 🌸

Thank you for having us !

Celebrating Physician Associates - and Eric Shelton

Physician Associates are an important part of today's healthcare team. PAs evaluate symptoms, diagnose and treat illness, order and interpret tests, manage medications, provide preventive care, and collaborate with physicians and other healthcare professionals to keep care moving forward.

In mobile medicine, that role can be especially valuable - helping bring skilled clinical care directly to patients who may have difficulty getting to a traditional office.

During National PA Week, Mobile Medical is proud to recognize the dedication, clinical expertise, and patient-centered care PAs bring to the people and communities they serve.

A special thank you to our very own Eric Shelton for the care and commitment he brings to Mobile Medical patients every day.

Join us in thanking Eric - and PAs everywhere - for the difference they make for patients and families.
