⭐ Improvement work spotlight: Simple change within the Acute Medicine Unit increases medicines reconciliation within 24 hours from 61% to 79% ⭐
We’re celebrating the work of MaryAnn Ling and the Wexham Park Pharmacy team following a quality improvement project across the Acute Medicine Green and Blue areas. 👏
The team set out to improve the number of patients receiving a completed Level 2 medicines reconciliation within 24 hours of admission to the Acute Medical Unit (AMU), supporting safer prescribing and alignment with national guidance. 💊
Using the Plan, Do, Study, Act (PDSA) approach, the team tested a simple change to their existing pharmacy prioritisation process.
Alongside high-risk patients and medication orders, pharmacy staff began prioritising patients admitted to AMU within the previous 24 hours, before moving on to patients with the longest length of stay.
Data from the four-week pilot in May 2026 was compared with a four-week control period. The results were encouraging:
📈 18.3% increase in Level 2 medicines reconciliations completed within 24 hours (rising from 61.0% to 79.3%)
📆 76.2% of eligible patients admitted within the previous 24 hours had their medicines reconciliation completed by the end of the day
🕛 Pharmacy staff were able to review more patients within 24 hours
The new approach also reduced errors on discharge prescriptions, enabled medication supplies to be ordered more proactively and helped the pharmacy team keep on top of its workload rather than continually trying to catch up.
Following the success of the pilot, the team have agreed to adopt the change in practice across AMU. High-risk patients and medication orders will continue to take priority, while patients with a longer length of stay will be monitored and, where possible, prioritised by the next working day.
MaryAnn Ling, Project Lead, said: “This project proves that small changes in daily practice deliver a massive impact on patient safety. Looking forward to driving further service improvements across the NHS!”
It’s fantastic to see a simple change in pharmacy workflow delivering a clear improvement in medicines reconciliation compliance and supporting safer, more proactive patient care. Well done to everyone involved in testing, learning from and embedding this improvement. We look forward to seeing the positive impact continue! 🙌
