November is American Diabetes Month. Do you know your A1C?

An A1C test is a blood test that shows your average blood sugar level over the past three months. Regular A1C checks can help you and your care team understand whether your diabetes care plan is working.

If you have diabetes—or are concerned about your risk—ask your PHC care team when your A1C should be checked.

Looking for a pediatrician in East Des Moines?

Dr. Heidi Shreck is accepting new patients at PHC East Side Clinic. She provides care for newborns, children and teens, including:

• Well-child checkups
• Sick visits
• Vaccines
• School and sports physicals
• Ongoing care as your child grows

Iowa Medicaid and most private insurance plans are accepted. A sliding fee discount may also be available.

Iowa Medicaid is changing.

Starting December 1, certain Iowa Health and Wellness Plan members may need to meet new monthly requirements to enroll or keep their coverage.

If you receive Iowa Medicaid:

• Watch your mail for notices from Iowa HHS
• Make sure your contact information is current
• Read and respond to requests for information
• Visit the Iowa HHS website to learn whether the changes apply to you

What a night!

Together for Health brought 170 members of our community together to celebrate PHC’s 45th anniversary and support access to care across Central Iowa.

Help our neighbors stay warm this winter.

PHC’s Homeless Support Services team is collecting cold-weather necessities for adults experiencing homelessness.

Items currently needed:

• Winter coats in adult sizes, especially men’s sizes
• Thermal underwear or long johns in adult sizes
• Winter gloves
• Warm hats

New or gently used items are welcome. Please make sure all donated items are clean and in good condition.

Medicare Open Enrollment begins today.

From October 15 through December 7, people with Medicare can review and make changes to their health and prescription drug coverage for 2027.

Before choosing a plan:

- Review your medications and health care needs.
- Compare premiums and out-of-pocket costs.
- Confirm that your providers and pharmacies are in the plan’s network.

PHC provides primary care and internal medicine for adults, including support for ongoing and complex health needs. PHC accepts Medicare, but coverage and network participation can vary by plan.
