Sometimes the best way to make a face look younger has nothing to do with adding volume.

Look at this real patient result following Nano-Ray laser resurfacing.

The most noticeable changes are in the appearance of brown spots, uneven pigmentation, and skin texture.

These are concerns that fillers and wrinkle relaxing treatments can’t correct.

Physician judgment is key here as not all dark pigmentation is the same.

Sunspots, melasma, and post-inflammatory pigmentation can require very different approaches. Some pigmented lesions need medical evaluation before any cosmetic treatment.

Skin type, pigmentation pattern, treatment depth, and the potential for post-inflammatory hyperpigmentation must all be considered.

My approach involves identifying the actual problem and creating a long term plan to treat the root cause.

Sometimes the answer is laser resurfacing. Sometimes it’s chemical peels, topical therapy, or no treatment at all.

I address what is actually bothering you, rather than simply recommending the latest aesthetic device.

COMMENT “BOOK” and I’ll send you a link to my guide on fearless aging.

Can you get Botox, fillers, and other cosmetic treatments and still age naturally?

I believe you can.

We’ve somehow turned aging into two extremes. Either you do absolutely nothing, or you pursue every new aesthetic treatment that promises to make you look younger.

But there is a much smarter middle ground.

In my book, The Master Strategist, I discuss an approach I call proactive, subtle maintenance.

It starts with understanding what is actually changing as your face ages.

For some patients, the priority is skin quality. Appropriate skincare, chemical peels, or laser resurfacing may help.

For others, strategically placed dermal filler can restore facial support without creating an overfilled appearance.

And sometimes, a little Botox is all that’s needed to soften expression lines.

The important thing is to choose the right treatments, at the right time, for the right reasons.

Enhance, don’t alter.

Refine, don’t inflate.

That’s my philosophy on aging naturally.

Collagen banking in your 40s. Is it smart anti-aging or just another skincare trend?

I actually like the concept, even though the name is a little ridiculous.

Collagen is one of the structural proteins responsible for skin firmness and elasticity. As we age, collagen production changes and existing collagen breaks down.

For women, the hormonal changes associated with perimenopause and menopause can accelerate that process.

The result can be thinner skin, fine wrinkles, crepey texture, and reduced firmness.

But collagen banking does not mean storing extra collagen for retirement.

It means protecting existing collagen and strategically stimulating new collagen production when appropriate.

I discuss my approach in this video. You can see why individualized treatment planning is so important.

Not everyone in their 40s needs the same treatment. Some patients need better skincare. Others may benefit from collagen stimulation. And some are actually dealing with volume loss rather than a collagen problem.

The goal is to preserve and improve skin quality before resorting to unnecessary treatments.

That’s the difference between aging strategically and simply chasing the latest trend.

The wrong laser can make brown spots and hyperpigmentation worse.

Not all dark spots are the same.

The best treatment starts with the correct diagnosis, not the most popular laser.

If I were 55 with just $2,000 a year for aesthetic treatments, I might tell myself to save most of it. Here’s why:

Facial aging at 55 can involve volume loss, collagen depletion, skin laxity, deeper wrinkles, and changes in skin texture.

These concerns often require different treatments.

Fillers can restore lost support. RF microneedling can stimulate collagen and improve certain types of skin laxity. Laser resurfacing can improve wrinkles, texture, and sun damage.

But $2,000 may not be enough to address all of these problems adequately.

These are not hard and fast rules. If your budget is more, or less, the plan obviously changes.

And, age alone doesn't determine what you need.

If I were 45 with just $2,000 a year to spend on looking younger, I would not spread that money across five different cosmetic treatments.

I would prioritize the concerns that make the biggest difference.

What would I skip?

Trendy treatments that don’t address my concerns, and discounted procedures that provide too little treatment to deliver meaningful improvement.

The best anti-aging treatment at 45 depends on your anatomy, skin quality, facial aging pattern, and what bothers you most.

That’s why I believe in a full facial assessment before recommending a product or device.

The best aesthetic investment isn’t always the most expensive treatment. It’s the treatment that addresses the right problem.

And if I were 55 with the same $2,000 budget? My priorities would change.

Should I break that down next?

Sculptra vs dermal filler vs facial fat transfer. Which is best for facial volume loss?

I prefer using Hyaluronic Acid Fillers when I need immediate, targeted structural support and precise control over shape and proportion.

Sculptra is different. It gradually stimulates collagen, making it useful when the change is broader and the goal is progressive remodeling rather than precise shaping.

Facial fat transfer can replace larger amounts of lost volume using your own tissue, but it is a surgical procedure with more recovery, and fat retention is less predictable.

None is universally “more natural.”

The most natural result comes from correctly identifying the problem and choosing the right tool, or combination of tools, to solve it.

That is also why I personally assess and perform injectable treatments in my practice. The decision about what not to inject can be just as important as knowing how to inject it.

I’m all about restoring what aging changed without changing what makes you look like you.

And there is one area where patients frequently ask me for Sculptra that I usually would not treat with it...

The FDA just updated the Kybella warning, and anyone considering fat-dissolving injections should understand what changed.

Kybella, or deoxycholic acid, is FDA approved for moderate to severe fat under the chin in adults.

It is not FDA approved for fat reduction in the abdomen, arms, jowls, under-eyes, thighs, or other areas of the body.

The FDA has now specifically recommended against these unapproved uses after reviewing reports of serious adverse events outside the submental area.

This does not mean Kybella is suddenly a bad treatment.

It means patient selection, anatomy, indication, injection location, and physician judgment matter.

One of the biggest mistakes in aesthetic medicine is assuming that because a treatment works in one part of the body, it can simply be moved somewhere else.

At my practice, I personally evaluate and perform injectable treatments because deciding whether something should be injected is just as important as knowing how to inject it.

And if the “fat dissolver” being offered to you is not actually FDA-approved Kybella or FDA-approved deoxycholic acid, there is another layer of risk entirely.

Aesthetic medicine has a commoditization problem.

Botox becomes “$X per unit.”

Filler becomes “one syringe for $X.”

Energy-based treatments become “300 shots for $X.”

But patients are not interchangeable, and neither are treatment plans.

The amount of treatment someone needs depends on their anatomy, the area being treated, their degree of aging or laxity, and what we are actually trying to accomplish.

When price starts determining the treatment plan instead of the patient determining the treatment plan, undertreatment becomes much more likely.

Then the patient gets a disappointing result and concludes the treatment itself does not work.

That is how perfectly reasonable technologies can develop terrible reputations.

Aesthetic medicine should start with an assessment and a goal, not a menu.

“How many shots do I get?”

It sounds like a reasonable question.

But when it comes to energy-based aesthetic treatments, the number by itself tells you very little.

A larger treatment area may require more coverage. Different degrees of laxity may require different approaches. Placement, treatment density, energy settings, anatomy, and the desired result all matter.

So I would rather have a patient ask:

“What do you think I need to appropriately treat this area?”

The treatment plan should determine the number of shots.

The number of shots should not determine the treatment plan.

And that distinction can completely change what you are actually getting for your money.
