April 16th is the Canadian National Advance Care Planning Day. It is a day for you and others to reflect on decisions you would like made about your health care, should you be unable to speak for yourself. Advance Care Planning Day provides an opportunity to open up a conversation with a loved one about their wishes for end of life care - or it might remind you to review your own plan.
