We believe in Advance Care Planning at Knott den Hollander Law. Tomorrow (April 16) is Advance Care Planning Day. Check out Resources. Contact us for assistance.
INVITATION:
Dr. Bill Lombard, long-time nephrologist (retired) in our county, volunteers with a team I've organized. His presentation, "The Realities of Advanced Medical Interventions," supports good decision making before a person tackles their AD. We followup REALITIES' talks a week or so later with volunteer ACP workshops. Our ACP workshop volunteer leaders are trained, certified facilitators.
On March 10, the day before ALL of our Realities' presentations and f/u ACP workshops were canceled for the foreseeable future, Dr. Lombard's talk was video recorded at Western Washington University -- those attending were WWU's RN-to-BSN students, faculty, support staff and community.
Dr. Lombard and our team invite anyone who wishes to use the video in their ACP efforts, to be our guest! Additionally, use his PP slides as a starting point, to design your own Realities' iteration.

Our model (Realities with f/u workshop) has resulted in more verifiably completed Advance Directives than have been accomplished by a major hospital group serving our community. At Realities, we give each person a copy of the Honoring Choices PNW advance directive for their review...in preparation for the f/u workshop.
Over the years, hundreds (of the thousands) who've seen Realities, have told us, "I had already done my advance directive, but came out of curiosity...now I see I need to re-do it, because I didn't understand what I was choosing!

Thank you, Advance Care Planning, for normalizing talking about, planning and prepare for end of life! Only then can we live life to the fullest! Geordie, Jasper and I look forward to connecting in 2020! Merry Christmas to all! :)
Several years ago, after my husband's death, two ICU nurses and I decided to raise awareness regarding what advanced medical interventions actually mean for the patient (and families) in the short and the long term. I co-produced this short video "Have the Conversation" with a local filmmaker -- it highlights experiences from three points-of-view about the critical importance of Advance Care Planning. "It always seems too early, before it's too late.:

Phyllis Shacter, author of "Choosing to Die: A Personal Story", is an educator on the subject of Voluntarily Stopping Eating and Drinking (VSED). She works to share the story of her husband's choice to experience a conscious death and avoid the dreadful late stages of Alzheimer's Disease. On her website and in her book, she educates the reader on VSED as a lesser known choice to consider for those who have been diagnosed with a terminal or degenerative disease that will greatly affect their quality of life as death nears. Personal information and facts on VSED are especially useful for health professionals and those who facilitate conversations surrounding end-of-life options. Visit the website, like our page, watch Phyllis's Tedx talk, or read the book (available for purchase on Amazon) to learn more about VSED.
